I would like my colleagues to be able to report an incident when something happens at their location. Could you propose a template for me? Would it be possible to make sure that only the prevention team has access to the form?

Yes, that is possible in Ishtar.List. I have checked that you are an administrator and that a Prevention group exists. I suggest creating a separate list and form called “Incident report”.
Access: there are two ways
Recommended

Colleagues may report an incident, but only Prevention may view and follow up the reports.

make the form available for submitting a report
enable record-level permissions
give the Prevention group at least EDIT rights
make sure that reporters cannot view the full list themselves

Only Prevention has access to the form.

Colleagues then cannot submit a report themselves, unless a separate reporting channel or portal form is provided.

My recommendation: option 1, with an open and simple reporting form for colleagues and a protected follow-up list for Prevention. That way employees can report without gaining access to sensitive incident information.

I have not created anything yet. If you agree, I can build the Incident report form with these fields and set up access for the Prevention group.
